UPDATE FOR VISITATION

Effective Wednesday, September 29, 2021

As you are aware, there continues to be an uptick in COVID-19 cases in our county, state, as well as nationwide.? This latest surge also includes a variant that is stronger and can cause more sickness.? Thank you for your patience and cooperation as we strive to keep all residents and staff safe from this virus!? All families and friends are encouraged to be fully vaccinated.

 

The good news is there have not been any recent exposures or positives since September 13th.??

 

Visitation is daily from 10am-3pm. Please call to schedule your visits!? Only outdoor visitation is permitted in the courtyard or the front entrance area, weather permitting.? No visitors may go down through the individual nursing units/hallways unless specifically directed i.e. Hospice or Compassionate Caregiving visits. The appropriate PPE (i.e., masks) MUST be worn at all times inside and outside.

 

Visitors for the 200 Rehab Unit please contact the front desk for further direction at? (570) 433-3161.?

 

Leaves of Absence (LOA) / social outings will continue as scheduled.? Please remember family must be trained and sign both the LOA form and the COVID-19 risk form prior to leaving the facility.? Facility staff will bring your loved one to the front desk.?

 

ALL VISITORS MUST SCREEN IN AND OUT AT THE KIOSK, INCLUDING CHILDREN, AND ALL VISITORS MUST WEAR SURGICAL MASKS

??? ??You must wear the printed name badge from the kiosk at all times

????? Children?s masks are available at the front desk; No cloth masks are permitted

????? Please adhere to social distancing guidelines

????? Infection control guidelines will be monitored by facility staff in all resident areas

 

IF YOU ARE BRINGING MEALS FOR YOUR LOVED ONES, PLEASE NOTIFY THE FRONT DESK TO ENSURE NURSING AND DIETARY ARE AWARE.? Please understand that visiting during mealtimes could pose an issue.

 

 

Protocol for Taking a Resident Out for a Leave of Absence (LOA) / Outing

The following steps must take place prior:

  1. An approved LOA form MUST be issued from the Therapy Department and signed before each outing.
  2. A COVID-19 Risk educational form MUST be signed by both the family member AND resident before each outing.
  3. The resident MUST be signed out at the kiosk in the main lobby before exiting and reentering the building.

The COVID-19 Booster Vaccine Clinics will be scheduled soon with our new Diamond Pharmacy partner.? Permission will be obtained for each resident from the Responsible Party.
